NOTICE OF ACTION FOR TERMINATION OF PARENTAL RIGHTS To: John Doe and Jane Doe, unknown birth parets ADDRESSES: Unknown

YOU ARE NOTIFIED that an action has been filed against you and that you are required to serve a copy of your written defenses, if any, to it on Melissa A. Posey. P.A., whose address is 201 E. Government Street, Pensacola, Florida 32502 on or before April 21, 2008, and file the original with the clerk of this Court at M.C. Blanchard Judicial Center, 190 Government Center, Pensacola, Florida 32502 before service on Petitioner or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the petition.

Copies of all court documents in this case, including orders, are available at the Clerk of the Circuit Court's office. You may review these documents upon request.

You must keep the Clerk of the Circuit Court's office notified of your current address. (You may file Notice of Current Address, Florida Supreme Court Approved Family Law Form 12.915.) Future papers in this lawsuit will be mailed to the address on record at the clerk's office.

NOTICE TO BIRTH PARENTS CONCERNING SAFE HAVEN SURRENDERED INFANT, BABY BOY DOE, DATE OF BIRTH: DECEMBER 8, 2007

Pursuant to Florida Statutes 385.50(6), a new born infant may be surrendered at a hospital. Baby Boy Doe was surrendered on December 9, 2007. Custody has been given to Catholic Charities of Northwest Florida, Inc. by Order of the Circuit Court of Escambia County. This publication is being made to notify the birth parents that this child has been placed for adoption. This publication is being made under Florida Statues, Chapter 49.011(10) to provide substitute service on the birth mother and birth father of Baby Boy Doe whose date of birth is believed to be December 8, 2007. Surrender of Baby Boy Doe was made at Baptist Hospital in Gulf Breeze, Santa Rosa County, Florida. This notice is the only notification that will be provided that a petition to terminate your parental rights as the birth mother, birth father and/or legal father of said child has been filed with the Escambia County Circuit Court and is currently pending. Because your name description and address are unknown, this publication will be your only notice that legal action is being taken to terminate your parental rights. According to Florida Statutes, Chapter 49.09, the return day for service upon the unknown birth mother and unknown birth father is the first date of publication of this notice. As a birth father you have certain rights to assert paternity. Within 30 days of the first date of publication, you must file an Affidavit of Commitment that includes submission to the jurisdiction of the court for support to be ordered, upon paternity being established; and, a written plan as to how you will care for the child if you assert your parental rights. Within 20 days of the first date of publication, you must file an answer to the Petition to Terminate Parental Rights with the Circuit Court of Escambia County. Any defenses, objections or answers should be filed with the Circuit Court of the 1st Judicial Circuit, in and for Escambia County, located at Escambia County Circuit Family Law Clerk, M.C. Blanchard Judicial Center, 190 Governmental Center, Pensacola, Florida 32502, under the case number at the top of this notice. Please contact the undersigned if you wish to sign consent to the adoption and terminate your parental rights without further legal action. If you choose to assert any inchoate parental rights, please complete the Florida Putative Father Registry. You may do so by going to the website for Florida's Department of Health, http://www.doh.state.fl.us/. If you do not own a computer, you may be provided access to a computer at public offices in your area such as the public library. Please advise my office if you are a member of or qualify to be a member of an Indian or Native American Tribe.
